Our Caregiving Team

Doctor and Patient
Attending Physician

An Attending Physician at our hospice is a physician who certifies that the patient has a terminal illness, and works as a part of the hospice team to provide compassionate and comprehensive care. The Attending Physician can remain the patient's primary physician, while also helping to coordinate care with the other members of the hospice team.

Checking Blood Pressure
Medical Director

Our Medical Director certifies the attending physicians prognosis and leads the hospice team to develop a plan of care tailored to each patient's individual needs. They also provide consultation to the attending physician to ensure the highest quality of care.

Seniors Socializing
Skilled Nurses: RNs & LVNs

Our skilled nurses provide the highest quality of care and support to patients and their families. They are knowledgeable and experienced in the field of hospice care, and have been extensively trained to evaluate a patient's condition during regular scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques, the use of medication, and effective pain management. Our skilled nurses are dedicated to providing compassionate and quality care to all of our patients.

Attentive Therapist
Social Worker

Our social workers are invaluable members of the hospice team. They assess the social needs of the patient and family, provide psychological and bereavement counseling, and make referrals to other services or community resources. They also provide emotional and spiritual support to the patient and family in order to promote a sense of well-being during this difficult time.

After Church
Chaplain

Our chaplain is available to provide emotional and spiritual support for both the patient and their family. This may include prayer, meditation, and spiritual reflection. Our Chaplain can also help coordinate with the patient's clergy or spiritual leader to ensure that the patient's spiritual needs are met.
